"Fresh" means something that is new, recently made or harvested, while "refreshed" means something that has regained freshness, or energy. Ex: In the morning when I wake up, I feel fresh, and in the afternoon, I take a nap to refresh myself. Ex: The vegetables are fresh from the garden, I'll put them in some cool water to refresh them before cooking them.
