The reason that the word applies to more than just literal rock stars (celebrity rock musicians) is because of the worldwide reach of rock music in the mid-to-late 20th century, which allowed musicians to reach global audiences easily for the first time and become very wealthy and famous. The negative use of the word for someone leading a self-destructive lifestyle comes from the association between rock music and such a lifestyle - in this cases, we can say that someone is "living like a rockstar". There is an additional meaning as well. Someone can be called a "rockstar" by someone else to encourage them or congratulate them in the context of a public performance of some kind. Ex: Ever since my brother went to college, he's been living like a rockstar. All he does is party. Ex: When you were singing on stage, the audience loved you. You're a rockstar!
