"At last" means finally or after a long time. You can use it when you've been hoping for or waiting for something for a long time. It has a feeling or nuance of some kind of relief that that thing finally happened. Ex: You came home, at last! Ex: And, at last, the weather started to get warmer. Warm enough to go swimming at the beach. Ex: She went to say hi to everyone, then, at last, she came and greeted our table.

Excellent question. "Difference in" is usually used to refer to a change in one thing, instead of noting the dissimilarity between two things. In this case the thing is a list of things. So a "difference in how you feel", "a difference in how you think" etc. Ex: I noticed a difference in the way you play piano now. (How you play piano gas changed).
