"Loaded" is used to mean full, so a "loaded pistol" is a gun which has bullets in it. It is not an idiom, rather here it is being used as a metaphor to symbolise that the woman this narration is about, is just as dangerous as a gun with bullets. Ex: It's dangerous to leave a loaded gun lying around. Ex: I want to make loaded potato skins with bacon and cheese.
