In the video, you will notice the "Santa" attempting to secretly pass the little boy some money, but the boy failing to catch it as his hands were too small. The speaker says "it would've worked this time if his hands weren't so damn small" to mean that his action would have been successful had the boy's hands been bigger. "Would've worked this time" can be roughly understood as "[x action] would have been been successful/effective." Ex: We could've won the game if the goalie blocked the goal. Ex: I would've arrived on time to school if I didn't oversleep my alarm.
