"Mixed up" is a term used to mean "confused" or "bewildered." You will notice considering context clues (i.e. the dog spinning around in his wheelchair and rolling his eyes around in his head), he is likely expressing that he feels dizzy or bewildered after all the action. Ex: I have a lot of personal problems today, I feel a bit mixed up. Ex: Wow, that rollercoaster ride got me all mixed up.
