Not always. The term "kiddo" is typically used as a nickname for children. However, it can be used towards adults who are significantly younger than another as a term of endearment. Typically used in familial relationships but it is not restricted to that. For example, my aunts will sometimes call me "kiddo" even though I am an adult.
