Is "fellow" only used for men? If so, what do you say in case of referring to "women"?

Yes, the term "fellow" is only used for males, either men or boys. If you wanted to refer to a female, either a girl or a woman with the same kind of casualness, you could use the words "girl" or "gal". Ex: She's a very pretty gal. Ex: A very intelligent little girl.
