Each of these are different terms used in film. To "rack focus" means to change the focus of a camera lens in a a shot, so something that was once blurred becomes clear and in focus. To "focus" on something here means to make the camera lens become clear on one object or person. This is different from "rack focus", as "rack focus" describes the action of changing the focus. Finally to "tighten" on something means to zoom the camera towards that object or person so that it is extremely close up.
