The question "what's not to understand?" expresses the meaning that someone should be able to easily understand something but for some reason, they still don't understand. It can also be used as a response when someone doesn't want to do something. Ex: I've explained this math problem ten times. What's not to understand? Ex: What's not to understand? You have to go to school. It's the law.
