Is using "by" and "near" at the same time grammatically incorrect? Or, could I use either of the one or even both?

Yes, you actually cannot use "by" and "near" together as they mean the same thing, so it is both superfluous and grammatically incorrect. You can choose to use of the two, but not both together at the same time! Ex: I will go to the supermarket near my house. = I will go to the supermarket by my house. If you want to use the word "nearby" to express that something is in close physical proximity, you can do so. Ex: My house is nearby. Ex: My office is nearby.
