A surrogate mother is a woman who has a child on behalf of another woman. It can also be used to refer to someone who acts like a mother to another person instead of their biological mother. It is quite a commonly used term. In this context, it is being used as a joke because the actress being interviewed has just had her third child; so when she gets recommended to have a fourth child, she is turning down the recommendation by saying she will only have her fourth child if she has the baby for her.
