Can I assume "get five balls in the basket" is skipped after "to"? Like "First person to get five balls in the basket".

Yes. When Leonard says "First person to five wins the office" he does leave out "balls in the basket." Later in the video, you hear Sheldon ask the question "Of what?" and Leonard replies "Balls in the basket." You may already know this, but sometimes in English, words or phrases are left out. This may be because it is implied, it was said previously, or it was taken out because the remainder of the sentence can stand on its own. Here is an example of a conversation where something is left out. A: How many apples would you like? B: I will have four (apples) please.
