Can I omit "a death" here? Does it still same mean without it?

Grammatically speaking, you could omit "a death" here. However, "death by a thousand cuts" is an expression that is never really used without "death". So it would sound less natural to remove "death" here because of this expression, and it would sound better to keep the sentence as it is.
