In this context, to "remind" somebody of something means to cause that person to remember something, likely because they are similar in some way. In this case, his ex-love causes him to think of the moon, which may be a metaphor to say that she is something he longs for but cannot get. Ex: You remind me of my friend Lisa! You guys have the same great sense of humor. Ex: My brother reminds me a lot of my dad. They have very similar hobbies.
