The origin of the phrase "ladies and gentlemen" comes from another phrase "gentlemen and gentlewomen" in the 1500s. It didn't refer to everyone. It referred to a group of people. "Gentlemen" and "gentlewomen" were people of higher social standing. And, now, it can be used quite casually as well as formally to refer to most people. Ex: Ladies and gents, please take your seats. Ex: The gentlewomen and gentlemen in the 1500s were considered of higher social status.
