Would it sound awkward if I say "condition" instead of "health" in this context? If not, are these words interchangeable?

It would sound awkward! "Condition" may imply a sickness, in which case, "mental illness" may be more appropriate. The term "mental health" doesn't always imply one of these illnesses. It could just mean caring for your mental health so that you don't eventually get one of these illnesses, or to just take better care of yourself in general. It's the same with "physical health", we exercise and eat well to stay healthy, but it doesn't mean that we're necessarily sick. Ex: I took a day off for my mental health. I felt like I was about to get burnt out from work. Ex: She likes to journal and meditate to help with her mental health.
