"How" and "why" are very similar here, but the nuance would be changed slightly if "how" was replaced with "why". "How" in this case is used more as a rhetorical question that he is asking himself, and so he isn't really expecting a reason but he is examining how he hasn't ever thought of something like that. If you replaced "how" with "why", it would imply that he is wanting a reason as to why he hasn't thought of that.
