When we talk about "Heaven" and "Hell," first letter is always capitalized without any articles?

They're not always capitalized! Here they're capitalized since they're actual places and considered proper nouns, but that's not always the case. There's not usually an article. Even if there is, you would use another word before them to define the type of heaven or hell it is. Ex: The religious hell and heaven is an interesting concept. Ex: I think my dog went to Heaven. Ex: Do you believe in heaven and hell?
