To "wear out" something means to use it until it looks old and used, or does no longer work properly. Thus, to be or look worn out means to look used, old, or deteriorated. Ex: This was my favorite shirt. I wore it everyday, so now it looks worn out and old. Ex: This playground has been around for decades, so it looks quite worn out.
