"Nerve" aquí significa algo como "osadía" o "descaro", que ambas tienen connotación negativa. Cuando el emisor dice "with the nerve to doubt her", es como una reprimenda a la osadía de dudar de ella. El "with" se puede utilizar combinado con otros adjetivos. Ej: "With a confidence contradicting her young age, she stepped on the stage."
