In this context, "hold out" is a phrasal verb which means to hold something where someone else can reach it. The lyrics "hold out the palm of your hand" means to reach out your hand (facing up) so that he can reach it. Ex: Hold out your hand and close your eyes. I have a surprise for you. Ex: He held out the basketball so I could grab it. Ex: You need to hold it out a little further, it's still too far for me to reach.
