Great question. The phrase "take my hand" means reach out and clasp your hand with mine. It is a pretty common idiom. Not to be confused with "hold my hand" which means, take my hand and continue to hold onto it. "Take my hand" is generally neutral, but is often used for an act of movement, of joining or traveling together, as if the speaker is leading the other person somewhere. Also, if you had fallen down and someone was helping you up, they might say "take my hand." This means "grab my hand so I can help pull you up." Ex: Take my hand, I'll lead you to the exit. Ex: Take my hand. I won't let you fall.
